     If you need to contact the doctor for a medical problem after normal office hours, please call the Medical Society at 463-1763.  If a life threatening emergency has occurred, call 911.  The doctor is unable to provide routine prescription refills or make appointments.

     In general, fees charged in this office are comparable with other pediatricians in the

area.  We recognize the need for definite understanding regarding financial arrange-

ments.  The responsibility for the payment of the fee is the direct obligation of the parent or

guardian of the patient who brings the patient.  If you have any questions regarding fees, please feel free to call us.

PAYMENT IS REQUESTED FOR ALL SERVICES AT THE TIME THE SERVICE IS

RENDERED

If you have insurance, we will file the claim for you, however, you are responsible for any co-pays at the time of service. Most major insurances are accepted including Caresource and Molina. Call for details.

     If your insurance company requires a written referral from us for treatment by an-

other specialist, we will ask that you notify our office five working days in advance of your

appointment with the specialist.  This allows us time to obtain approval, prepare the referral,

and mail or fax accordingly.  Most insurance companies will not allow us to “back date” a

referral.

Frequently Asked Questions

     When you arrive at the office for a scheduled appointment, we will verify your address, telephone number and insurance information.  Please notify us immediately of any changes in insurance or address.  Please bring your insurance card to each visit.  Once a year we ask that you sign a new registration form.  This gives us permission to treat and file the claim and other required information with your insurance company.

     We ask that you request prescription refills at the time of your regular visit whenever

possible.  If you need a refill before your next appointment, call our office and be prepared to

give patients name, name of medication, number of times medication is taken per day,

and the name and telephone number of your pharmacy as well as a number where you can

be reached.  Refills will be called in by the end of office hours.

     Prescriptions cannot be refilled over the phone , if you have not seen the doctor in the last

six months,

If it is for a new illness or for antibiotics, please call the office to schedule an appointment.

     If you are unable to keep an appointment, we ask that you cancel at least 24 hours in advance.

If this is not possible, call as soon as you can so that another patient can utilize your appointment

time. 

You may be charged a cancellation fee if notice is not provided. This can not be billed

to your  insurance.
